Terms of Service Agreement

By booking an appointment or receiving care from Fillmore Chiropractic, you acknowledge and agree to the following:

Nature of Care
Chiropractic care may include examinations, spinal or extremity adjustments, soft tissue work, and related therapeutic services. Chiropractic care is not a substitute for medical care, and no medical diagnosis or guarantee of results is implied.

No Guarantee of Results
Individual results vary. No guarantees or warranties are made regarding pain relief, recovery time, or treatment outcomes.

Informed Consent
You confirm that you have provided accurate and complete health information and understand that chiropractic treatment may involve risks such as temporary soreness or stiffness. You consent to treatment by licensed providers at Fillmore Chiropractic and understand you may withdraw consent at any time.

Payment Policy
Payment is due at the time of service unless otherwise agreed upon in writing. Cash pricing, care plans, or prepaid packages may be offered with terms disclosed at purchase.

Appointments & Cancellations
Appointments must be canceled or rescheduled at least [24 hours] in advance. Late cancellations or no-shows may result in a fee of $50. Repeated missed appointments may result in restricted booking access.

Prepaid Packages
Prepaid packages are non-refundable unless required by law. Packages may have expiration dates. Unused visits are not transferable unless stated otherwise.

Communication Consent
By providing your phone number or email, you agree to receive appointment reminders, care-related messages, and occasional clinic updates. You may opt out of non-essential communications at any time.

Privacy
Your personal and health information is handled in accordance with HIPAA and applicable state laws.

Limitation of Liability
To the fullest extent permitted by law, Dr. Brendan Saffron Fillmore Chiropractic and its providers are not liable for indirect or consequential damages related to services provided.

Governing Law
These terms are governed by the laws of the State of Utah.
